Simply complete our secure online referral form, providing essential patient details, the reason for the referral, and any relevant medical or dental history. To ensure comprehensive care, we encourage you to attach any supporting documents such as radiographs, clinical notes, or treatment records. Once everything is prepared, referrals can be submitted through our secure portal or emailed directly to KidzyDental@gmail.com
